Annual Back to School Update

The Duxbury Public School District requires that you complete the Student Back to School Update for all your child(ren) every school year.  This annual update includes essential information about your child(ren), including our yearly health update and necessary permissions. Our school nurses can not provide over-the-counter medicines to your child(ren) until this form is completed. Additionally, you will be asked to sign off on School Handbooks, Social Media Policy, and the ability to grant permission or opt-out to Third Party Vendors, College, and Military Recruiters. This information is updated using your Aspen Family portal account and selecting the Health, Permissions & Bus tab.
