Pre-Participation Head Injury Form - Only fill out this form if your child has sustained a prior head injury. 

Report of Head Injury Injury Form (In Season) -  Please complete this form if your child has a head injury outside of school related to extracurricular athletic activities during the sports season.  In the event that the injury occurs during a Duxbury High School athletic event, the coach will fill out this form with the help of our athletic trainer, Tim McPhillips.

Mandatory Clearance (Return to Play) Form - This form must be submitted in order to return to play. 

NFHS Parents Guide to Concussions
The National Federation of High Schools (NFHS) has posted an informative brochure for parents about the symptoms, risks, and management of concussions.

The athletic department has a separate insurance policy to help in the case of injuries sustained during practices/games.  Families are expected to exhaust their primary insurance before using the athletic department insurance.  Paperwork for our insurance is available through Tim McPhillips, our athletic trainer, and claims must be made within 90 days of the incident.
