• 5 doses DTP unless the 4th dose is given on or after the 4th birthday
    • 2 doses Varicella or a physician certified reliable history of chicken pox
    • 4 doses Polio unless the 3rd dose is given on or after the 4th birthday.
    • 2 doses MMR
    • 3 doses Hepatitis B
    • Documentation of Lead Screening Blood Testing
    Massachusetts General Laws Chapter 76, Section 15,
    Requires certification that all children attending school are fully immunized unless exempt for medical or religious reasons. Medical exemption forms must be signed by a doctor outlining reason for exemption. Religious exemption forms are available in the nurse's office. Both medical and religious exemptions forms must be renewed annually. Required immunizations include Diphtheria, Tetanus, Pertussis (DTP), Polio, Measles, Mumps Rubella (MMR), Varicella and Hepatitis B. If your child has had chicken pox, the doctor must provide certification and date of the disease. 

    FLU VACCINE - Although a flu vaccine is not required at this time, annual flu vaccines are highly recommended.
